Family membership consists of a maximum of 2 adults and unlimited children living in the same home

Plain Medicare plus a Supplemental Insurance (or Medicare Advantage Plan PPO)

Pairs well with a Direct Primary Care office

Allows for ordering of tests and placing referrals

Patient, rather than insurance, is billed directly by family physician

Medicare Advantage Plan HMOs are a disadvantage

HMOs restrict doctors’ orders.

Patients are forced to use an HMO-insurance-contracted provider instead of a direct primary care doctor.

HMOs also prevent people from seeing the specialist they want or need, even refusing to pay for the visit after it has taken place.

Initial visit fee of $75 per individual, $150 maximum for family signed up same day, combined with first thirty days fee for each member

First individual: $75 (joining fee) plus $90 (1^st month membership)

Second individual: $75 (joining fee) plus $70 (1^st month membership)

Third individual: $0 (joining fee) plus $50 (1^st month membership)

Fourth Individual: $0 (joining fee) plus $50 (1^st month membership)
