Initial visit fee of $75 per individual, $150 maximum for family signed up same day, combined with first thirty days fee for each member
First individual: $75 (joining fee) plus $90 (1st month membership)
Second individual: $75 (joining fee) plus $70 (1st month membership)
Third individual: $0 (joining fee) plus $50 (1st month membership)
Fourth Individual: $0 (joining fee) plus $50 (1st month membership)
