The office relocated from Blackberry Ln to Neff Ave on 10/06/2025. It is on the first floor of a two story building, Unit J, in the Mall Centre section of professional businesses.
Email the practice for general questions. Messages can also be left on the phone, as this phone line always goes directly to voicemail.
Before your on-line registration is confirmed, we’ll aim to have a Practice Inquiry phone call with the doctor to answer any other questions and determine whether this new style of doctor-patient medical relationship is best-suited for you.
Emails that follow confirmation may end up in spam and include:
a) paperwork to be printed, completed, and brought to the first visit
b) a self-scheduling link
c) a link to additional forms to complete found in the electronic medical record
d) a release of medical records form to complete and bring with you

With the Hint links below, enter payor’s name as the “Primary Member,” select “enrolled/enrolling” or “not enrolled” as a patient at Orchid Family Medicine. Then add additional family members as “Dependent” even if one is an adult. There are limited insurance selection drop-downs, so it is easier to select “No existing coverage” and have the office scan any insurance cards at the first visit. Insurance information is sometimes used when ordering tests.
